Unlockables
- The following can be unlocked by beating the game with the listed requisites.
Endings
- There are a total of five endings - to obtain all of them, you need to do multiple playthroughs. The endings, and how to obtain them, are as follows:
- Bogeyman Ending: Don't kill Alex's mother, don't forgive Alex's father, Don't save Wheeler.
- Drowning Ending: Kill Alex's mother, but don't forgive Alex's father.
- Good Ending: Kill Alex's mother, forgive Alex's father.
- Hospital Ending: Don't kill Alex's mother, but forgive Alex's father.
- UFO Ending: Don't kill Alex's mother, don't forgive Alex's father, save Wheeler.
Extra Costumes
- There are five extra outfits to unlock for Alex that are obtained with certain endings for the game:
- Pyramid Head Helmet: Get the "Bogeyman" Ending.
- Deputy Alex Outfit: Get the "UFO" Ending.
- Order Soldier Outfit: Get the "Drowning" Ending.
- Orderly Alex Outfit: Get the "Hospital" Ending.
- Trucker Alex Outfit: Get the "Good" Ending.
Secret Weapons
- There are two secret weapons that are unlocked after you beat the game once. Below are the weapons and there locations during your second playthrough:
- Circular Saw: In the garage at the Shepherd's family house.
- Laser Pistol: In Joshua's room in the Shepherd's family house, on his foot locker. Get the UFO Ending to unlock this.
