A Perfect Playthrough in Guns of the Patriots consists of the following:
- No alert phases
- No continues
- No kills
- No health items
- No "special items,"
- Beat the game in less than 5 hours
The perfect playthrough requirement can be completed on any difficulty level. However, if you achieve the necessary goals in hard or extreme mode, you receive Big Boss rank, unlocking the Patriot sub-machine gun. You can also receive the Big Boss Emblem by completing a perfect playthrough on the hardest difficulty, unlocking the Big Boss Mask.
