On April 15, 2006, Carmona made his Major League Baseball debut with the Cleveland Indians. In his first season, he appeared in 38 games and finished with a 5.43 earned run average. In 2007, Carmona had a breakout year and finished the season with 19 wins and an impressive 3.06 earned run average.
