TIP: Always equip the Shotgun before entering a door - you never know when an Imp is going to jump at you.
There's a Z-SEC waiting for you as you enter the door, and a Commando after him, followed closely by two more Z-SEC Zombies. Kill them all from the safety of the doorframe, then enter and grab the armor and Cells from the crevice on the left.
Go into the room on the right and use the control panel to unlock the door on the left, then enter it and go into the bathroom (first door on the right) to kill a Z-SEC and grab some armor and ammunition. Enter the Lower Terminal Office to find an Imp and a Commando, the latter of which spawns in the lower level of the room. Grab the PDA, and open the next door to fight a Z-SEC and a Commando while an Imp spawns behind you.
Enter the room across the hallway and use the Health Station if needed, then use the control panel to unlock the door to the teleportation chamber (outside the room to the left). Leave the room, kill the Imp and enter the Chamber Decontamination room to your left. Enter the door on the right, kill the two Imps inside, and unlock both security lockers with code 836. There's two doors here - take the one on the right, and kill the Commando that appears. Head through the next door to find another PDA and an Imp.
Backtrack to the room with the lockers and enter the other door, kill the Chaingun Commando that's on the upper level across the room. Head to the control room, kill the Commando that charges behind you, and activate the teleporter with the control panel. Choose the storage room destination and step on the teleporter in the center of the room.
You'll be attacked by an Imp as you materialize in the storage room - kill it, and unlock the security cabinet with code 841, exit the room, and turn left to return to the teleporter, finding and killing the Z-SEC on your way back. Choose the balcony location before teleporting again, and head into the Transfer Bay Access.
The exit for the level is on the left - head right instead, and enter the control room, kill the Imp, and use the control panel to unlock the transfer bay. On your right as you leave the control room, the wall will drop, and a Commando and Revenant will attack - kill them, then look out behind you for another Revenant that spawns in the control room. Exit to the next area.
There's two Imps that will attack as soon as you enter the hallway - kill them, enter the next room and open the left door to find a Revenant and a Commando. Step through the door, and you'll find a Chaingun Commando and two Imps - drop them, and head down the hallway to the next room, killing the Imp that drops.
The bridge is out, so head right - a Revenant will spawn in front of you, so blast it with your Shotgun and keep moving. The path with split here; take the left path into Chamber 3 Terminal and through the next door. Descend the ladder and keep your eyes out for an Imp that spawns on your right. There's armor in the next room, as well as health, Rockets, and a control panel for you to use. After you use the control panel, leave the room and kill the two Imps above you on the catwalk, then climb up to the catwalk and kill the Imp that jumps at you from the left. Backtrack through two rooms to return to the split path, and head down the left path to kill an Imp and enter the decontamination chamber.
Exit the decontamination chamber with a big gun out and kill the Commando and Chaingun Commando, which both charge you from across the room. Head into Teleport Station 3 and kill the Revenant that spawns on the right of the teleporter, then spin and kill the Imp that spawns behind you.
Use the controls for the teleporter and select the Pad 3 Storage Room for your destination, then step on the teleporter - you'll materialize at the beginning of the level, so stock up on supplies, exit the room, and kill the Imp. Head back to the teleporter, and choose the destination to Pad 2 Storage Room - there's a ton of Plasma Gun ammo here, and a PDA.
Backtrack to the teleporter and choose the Pad 1 Exit as the destination - before the exit is a hallway on your left that holds a ton of Chaingun ammunition, but there's a swarm of Trites and an Imp before you can backtrack to the exit. Head through the exit to complete the level.
Delta Labs Level 4

Delta Labs Level 4 Overview
NOTE: This is a short level - and I do mean short. Be thankful, though - immediately after this level, you'll have to fight your way through Hell itself.
As you move down the hall you'll see a Chainsaw Zombie - don't bother shooting it, it isn't real. Head to the next door, enter, and grab the PDA. Turn around and kill the real Chainsaw Zombie that's running down the hall after you.
Move down the hallway and through the next door to fight another Chainsaw Zombie to your left. The hallway splits here, so head down the left path into the bloody area with a security locker. Unlock it with code 579, and then exit the room to go through the door you passed before.
You'll be acquainted with Hellknights. You'll have to fight them as a pair, and they do quite a bit of damage, so I'd recommend using the BFG if you've got the ammo; you're going to lose all your weapons pretty soon, so don't be stingy. When they're dead, you'll be automatically taken to the next level.
