In the 2008 action-RPG Castle Crashers for Xbox Live Arcade, Insane Mode is unlocked after completing the game on regular mode with any character. How to Unlock Insane Mode

Beat the game with any character. When you enter the map, a new path will be available south of The Barbarian boss stage. Enter it to unlock Insane Mode.
