The police report that has now been released contains almost two pages of handwritten notes from the woman who claims to have been assaulted. She details the particular evening when she claims the event took place. She and her friends had apparently gone out for some drinks that evening and, at some point, meet up with NFL player Ben Roethlisberger. Together, all of them reportedly went club-hopping and eventually ended up at a VIP club. It was at this VIP club that the incident apparently occurred.

The police report was officially released on Thursday, April 16, 2010. In the report, the young woman who claimed to have been assaulted by Roethlisberger offers her own details about the night. A PDF version of this report was posted on the internet and made available for download at many news websites. The Milledgeville Police Department—located in the state of Georgia—is the agency who released the documents to the public and news agencies. Before the documents were released by the police department, however, the name of the female accuser was redacted. The form contains her handwriting and originally contained her name at the top and her signature at the bottom. Both of these sections of the document have been covered up by means of a black marker. This is a standard practice, used to protect the identity of the accuser from the public at large. The lady's name will likely remain unknown until the police investigation into the allegations has been completed or unless the case against Roethlisberger happens to go to trial.

Upon arrival at the nightclub, the accuser claims her arm was grabbed by one of Roethlisberger's staff. The person grabbing her arm has apparently been one of the NFL-player's hired bodyguards. Telling the accuser to "sit on a stool", the bodyguard then disappeared out of the room. After a period of time, Roethlisberger allegedly came into the room and, after having zipped down his pants, exposed himself to the young lady. At this point in time, the accuser tried to leave through a nearby door. The door was not an exit, however, and would have led the woman into a restroom. She apparently repeated her previous statement, "it was not OK," a number of times before Roethlisberger is alleged to have had sex with her.
