The meme got started on YouTube. A member on YouTube, Brad O'Farrell ( http://www.youtube.com/user/BradOFarrell ), uploaded the video. It was uploaded on February 2, 2009. He simply found and combined a fail video and the cat video to come up with this:

I just started seeing it within the last week and considering I am online practically 24/7 it's hard to believe this one has gotten by me for so long.
It appears the original is about 3 months old and the meme started rolling about a month ago...according to YouTube dates and that's all I got. I just typed in "play him off keyboard cat" into YouTube search then sorted by "Oldest"
And the first result has a two month difference before the rest start rolling in.

That first result is actually quite disturbing, it's a man in a wheelchair falling down and escalator, then being played off. The good news is, the YouTube credited this website for creating Keyboard Cat. http://charlieschmidt.com/
