There is not an actual Vault 77 in the game. It was created to narrate a story in the Penny Arcade web comic. The jumpsuit is the only item in the game referencing Vault 77.

It's a reference to a Penny Arcade comic series.

It is located in the Slaver barracks in Paradise Falls, on a shelf.

Some people think the jumpsuit is cursed and will make the Puppet Man attack during the game. This hasn't been proven though.
