The "Survivor" achievement takes time to get. You need to finish "arcade mode" with all 32 fighter planes. With each plane, you need to take down 36 planes in under 6 minutes for a total of 1152 planes.
"Colonel" is achieved in campaign mode. You need to earn the Colonel rank, which comes after lt. Colonel and Flight Officer. You should be able to earn this is you are playing the game for the second time through. Common thoughts are that you need around 700 plane kills, 700 Turrets/bunker/AA guns and 100 naval units destroyed.
