Would you consider the iPhone 4 antenna problem, as only a problem caused by users or as a shortcoming in design by Apple?

If holding the phone the wrong way makes it fail, then it is a design flaw. Users should be free to hold the phone in any position. This is Apple's fault - pure and simple.
