The top commenter's avatar either has slightly more padding than the following avatars or less characters set in the usernames truncate function.
This is to prevent long usernames breaking the page layout if they overflow.
Pixel perfect webdesign is a nightmare to perfect, especially with user submitted content so truncation and other techniques are needed to keep everything in check.

M$True. Either go around and look for a better solution (bigger avatar, thus bigger space for name) or put a limit on character number, then allow those with long username to change them.
Just slightly bigger avatar will be good enough to accommodate those with "too long" usernames. I would rather the right sidebar to be narrow down to make space for the avatar.
