Why is my 5-month-old son's pediatrician concerned about him "pointing" his toes?

At my son's appointment today, his doctor muttered something under her breath about him pointing his toes. At the time he was kind of writhing around on the table because he wasn't very happy about being examined.

Bit of background: He was 6 weeks early (emergency C-section), had to be resuscitated at birth, and spent 6 weeks in the NICU due to pulmonary hypertension and then weaning off of sedation meds and learning how to eat.
