They all seem to get away with violating basic sanitation rules by leaving both cooked and uncooked meats out at room temperature for hours. I've seen bacon cooked in the morning sit on top of the conviction oven from 9am until the next morning and being served all day long.
Soups and meat special items have set out on tables for hours because they didn't want to walk to the cooler or make room for them in heat wells.

I actually worked at Taco Bell so I know what ya mean :)
I worked at Taco Bell for 2 weeks. When I came home from work I smelt worse than when I worked Construction in 100 degree heat.
A lot of Fast Food is made without gloves or with dirty hands. When I worked at Taco Bell nobody changed their gloves, and they would wash dirty dishes in dirty water than not dry them off, and make people's burrito's. After working there I can't go to Taco Bell again. Also, you don't want to know how cheap and nasty there ingredients are.
