"The Soccer Ball appears on the levels, Foundry, Standoff, Rat's Nest, Avalanche, Ghost Town, Cold Storage, and Blackout as a Forge item"
So first you have to have the multiplayer maps that have the soccer ball in them. Then create a new map and it will be one of the items that you can spawn in.
