Where do I go to report a restaurant that serves food that is dangerous to the public?

I am not being picky, and I am not taking out a grudge. I am looking to report a restaurant that clearly serves food that is poisonous. I am not able to locate a FDA Web site form where I can report this.

I can seem to find some phone numbers here http://www.fda.gov/Safety/ReportaProblem/ConsumerComplaintCoordinators/default.htm however, I would prefer to document this in writing.

The phone call would likely be just your first step in the report, after you talk to someone you could probably request a more detailed way to contact someone... you should also consider trying to contact your local health department as they might be able to do more for you in person even.

great answer. defintely the local health department that does restaurant inspections.

Call the Health Department in your city or state. They should be able to explain you what to do and where you can find the information online.
