The Large Hadron Collider (LHC) was first activated in 2008 (see http://lhc-milestones.web.cern.ch/LHC-Milestones/year2008-en.html ) and ran for just over a week before a severe incident shut it down. The incident occurred when an imperfect solder joint failed, causing an explosive cryogen leak.
