Same goes for more complex and 3D or photo effects - usually designer takes the basic font, and applies effects in his/hers chosen application.
If you seek some specific effect, you may choose the font you like and ask a designer to apply or help you to apply that effect to the font - OR - you may just want to hire someone to do the whole searching for font and designing all together.
