M*A*S*H*, with Donald Sutherland and Elliot Gould. There is a scene where they're goofing about, then casualties come in. They pull the bandages off one soldier and are faced with a geyser of blood - apparently the bandage was compressing an artery. I went from giggling to crying for the guy in less than 2 seconds. Amazing.
