I wish I knew. Traditionally, in Biblical thought, there are distinctions between types of love (see C.S. Lewis' "The Four Loves" for more). Intuitively, most people distinguish between fraternal love and sexual love pretty easily. But there can also be a more universal love, something akin to charity, and love of things only.

Still, all of that logic runs into a deep problem: What makes all the different types of "love" love? The fact desire is involved? But that still raises more questions than answers: we don't consider all our desires "loves."

You can approach the problem of true love a number of ways. My two cents: Plato's Symposium is probably something you want to study (Seth Benardete's translation and commentary is highly recommended), and Emily Dickinson and Yeats are worth looking at. I think it depends on the person.

For me true love means (this isn't a complete list by any means):
Being without your SO is just heartbreaking
You would do anything for your SO
You are happy or elated to be in the company of your SO
When you think about them, nothing but happiness comes to mind
Knowing that come Hell or High Water, you can rely on them, and they can rely on you
Selfishness doesn't exist between the two of you
Communication isn't a problem
Neither of your hearts hold secrets to or about the other
