Lippman wrote the cfront preprocessor at Bell Labs, and his explanations of how things work and how to use them are very accessible. A good exposition of the theory and practice of object-oriented design as well.
Another good one is "C++ How to Program" by Dietel & Dietel. Lots of examples, clear prose, great book.
