What is an Apicoectomy & do I really need to be under anesthesia if I prefer not to? How long does procedure take?

I had an abcess in my mouth & was sent away from dentist with instructions to get this taken care of asap. I'm prone to panic attacks, expecially when strapped to a dentist chair but am PETRIFIED of anesthesia. Curious if anyone has had this done w/novacaine only

As a disclosure - the following is intended for informational purposes only. You should follow the advice of your dental expert (your dentist or endodontist) on this matter.

An apicoectomy is an endodontic procedure in which the end of a root of the tooth in question and any inflamed or infected bone tissue around it are surgically removed (see http://www.aae.org/patients/patientinfo/faqs/endosurgery.htm#2 for some graphics and a brief explanation).

According to http://www.1800dentist.com/dental-encyclopedia/apicoetomy the procedure takes between 30 and 90 minutes.

As far as the type of anesthetics, according to http://www.1800dentist.com/dental-encyclopedia/apicoetomy this is a special anesthetic with double the amount of epinephrine compared to usual dentistry anesthetics which helps by constricting blood vessels in the area, reducing bleeding, and making it easier for the endodontist to see what s/he is doing. This is contra-indicated if e.g. you suffer from high blood pressure.
