What exactly is a metrosexual? And does it work both ways? So are the really butch gay men considered metrosexuals too, just in reverse?

"Metrosexual is a neologism of the 2000s generally applied to heterosexual men with a strong concern for their appearance, and/or a lifestyle that displays attributes stereotypically related to gay men.
"...other terms countering or substituting for "metrosexual" appeared. Perhaps the most widely used was "retrosexual," a traditionally masculine man who rejects focus on physical appearance, sort of the opposite of a metrosexual.
"Etymologically, metrosexual comes from the Greek meter = mother and the Latin sexus = sex. Mark Simpson was unaware of this when he coined the word, but later endorsed the etymology, adding "It's post-Oedipal." " - http://en.wikipedia.org/wiki/Metrosexual
So by usage definition a metrosexual is applied to heterosexuals. If by "butch" you mean a gay man who rejects the focus on personal appearance it seems to fall under the definition of retrosexual but so far it's only used to define ungroomed straight men.
Breaking down the prefixes involved as used in reference to the question:
homo- same, think gay
hetero- opposite, think straight
metro- looks gay (due to a stereotypical focus on appearance attributed to gay men) but is straight
retro- ungroomed, masculine and rugged looking (typically used to describe straight men with little concern for personal appearance)
Short answer, no. "Butch" gay men are not retrosexual by current popular definition. Metrosexual is only used to categorize straight men.
