I'm afraid that - from my strictly amateur and limited perspective - the only real way that we found was to get his teeth professionally cleaned (expensive!!!) and then regularly (try to) brush his teeth ourselves.
We find that it is much easier when two or three people take on the task together. Ideally, one brushes, one gently pulls his lips/gums up, one gives him pats and reassures him that we're almost done!
