Looking back, the ancient Egyptians invented the first disposable tampons made from softened papyrus.The ancient Greeks created tampons made from lint wrapped around a small piece of wood, this was recorded by Hippocrates sometime in the fifth century B.C. But physicians in the medieval Islamic world also described the use of tampons, often for contraceptive purposes.

Other materials used for the first tampons have included: wool, paper, vegetable fibers, sponges, grass, and later cotton.

Wow, good question!

These answers seem pretty logical. I guess in the not too distant past they used rags and towel mostly.
