I'll assume you mean buying a DSLR camera used or second hand. The most important thing is the quality of the sensor. A well taken care of camera will have a clean sensor with no dust spots or splotches. There are ways to clean the sensor yourself -> http://www.pbase.com/copperhill/ccd_cleaning but severely neglected cameras will be hard to clean.
The second most important thing is to find out the number of shutter actuations, or the number of pictures that were taken. Shutters tend to wear out and the more times the shutter has been fired, the closer it is to breaking. Granted many DSLR's have shutters rated for several tens of thousands of shutter clicks, it is something to keep in mind.
After that, making sure everything works and is in good cosmetic condition. If the body has some dings and scratches don't worry about it as long as they don't interfere with the functioning of the camera. Check the LCD screen for any scratches and make sure all of the buttons work (i.e. aren't stuck or sticky).
If you look out for these few issues you will be in good shape when buying a second hand DSLR.
