There are two ways that two "simultaneous" blood clots can occur:
1. Deep vein thrombosis (see link) can be caused by things such as prolonged sitting, which can affect the blood flow in both legs equally and cause bilateral clots to form.
2. It is often asymptomatic, so if a clot in one leg is causing symptoms, then testing could reveal a clot in the other leg. The two clots might seem to be simultaneous because they were found at the same time, but they could have formed months or years apart.
