If you are asking for the special plates, it depends on state law. Usually what it means is that in order to qualify for a classic tag, you can only drive it for X miles per month, not counting things like going to club meets, special events, etc. They don't want you to take a gas guzzler, register it as a classic (which also means you don't have to do emissions testing) and then use it to commute 20,000 miles/year.

Here in Virginia it is called the "Antique" tag, and is restricted as follows:

"A vehicle must be over 25 years old and used primarily for antique exhibitions, parades, and related activities to qualify for antique status. (The vehicle may be used for infrequent pleasure trips of under 250 miles, and to obtain repair work.)"

You pay for the tag once, no need to renew it yearly. If you want to use that tag but not be bound to the mileage restrictions, they will happily let you have the tag as long as it is renewed yearly.
