My idea of a video game would involve a Fallout 3/Borderlands style FPS. It will have fully customizable settings, from facial features all the way to body mass and voice pitch. I always liked the idea of a one of a kind character.
I would also encorporate a crafting station within the game unlike any other game ever made. It will give you pieces of the armor/weapon you want to craft that way its fully customizable too. For instance, You want a triple barrelled shotgun, that is sawed off to appear like a big pistol, that one of the barrells shoots out grenades; you can customize that. In order to fully capture this idea, it would have to be in the form of a store that sells individual parts that you can upgrade your weapons to your own desire. It would also have to be a fantasy style crafting, as some guns would just be impossible to make in real life scenarios. I would even go as far as putting a paint like program in it, so that you can draw or craft your own class emblem to put on armors or even weapons you may make. To top it off, the safe house you recieve in the game will be fully customizable and one of a kind as well, almost like a Forge type feature. Basically hours upon hours of customization, if you choose too; otherwise you can choose generic.
The storyline would have to be post apocalyptic, but not a wasteland either (Borderlands, and Fallout 3 already did this). Instead, its storyline would take place on the humans last civilization, which would not be Earth. This civilization would be a mixture of all different terrains (Swamps, Plains, Forests, Mountains, Deserts, and even Undersea civilizations) to include different planets and through the questline you will be able to be a King or Queen over an entire planet, depending on which one you choose. This is just a rough idea of what I would like to make, not to give away the main questline and quests. This will definitely be a long game with an experience based leveling system. It will also be four player cooperative, and an in and out in-game join system.
Just an idea, and hopefully not a stolen one now that its out in the open lol. I claim this my idea lol.

To add a little more to my answer: There will be approximately 6 types of characters, all human with different skill trees based on preference (ranged, close quarters, medic, ect.). I would also incorporate a questline, that would branch off at a specific point depending on what alignment you choose (Evil characters and Good characters will have their own special additional skill tree to utilize per a level). The questline would be in the sort that you have a rival that would always be the opposite alignment as what you choose, and the end-game premesis would be to either stop him from world domination and control, or he will try to stop you from this. That way you see both sides of the spectrum, without being bored with the game after the first playthrough.

If you get the chance check out the movie - Phantasm 2. This guy fuses two sawed off double barrel shotguns together. Your idea reminded me of it.
