There isn't a ton that a heavy can do for spy defense. There are a few basics though. Just pay attention to what's going on around you. If an engineer is running toward you, put some bullets in him. If any other class is trying to run past you, and they have full health, shoot them. That's the easiest way to spot them. They don't have much health and you can't wound your teammates. A few quick shots should do wonders for spy detection.
If you find that spies keep sneaking up on you, then you need to change your strategy a bit. Try to team up with a Pyro (they're practically made for spy detection). Then just keep an eye on your back and stick with your little group. They can't take you all out at once.

In Team Fortress 2, what is the best way for a Heavy to defend against a Spy?

• Thanks for the info. It's definitely tricky to do, but shooting at people and having a Pyro around help.
