The answer to this depends entirely on taste, and on mood. Sometimes I like a good beach vacation. Other times, I want to go to a city to eat good food and sightsee. Some personal favorites:
1. Cities: Vancouver BC (great combination of urban amenities with spectacular natural scenery). NY NY (the city so nice, they named it twice!), Washington DC (for the excellent, and free, Smithsonian museums).
2. Tropical: Kona Village Resort, Big Island, Hawaii. All-inclusive, no electronics (no TV or phones in the rooms), private bay/beaches. Expensive (naturally). (http://www.konavillage.com).
3. Snorkeling: Cozumel, Mexico, is wonderful for snorkeling and diving. Amenities are also good, though the crowds from cruise ships can be a bit crazy.
4. International: Italy. Stunning art everywhere, fantastic food, good public transit. Learn a little Italian and everyone will treat you really well. Go slightly off season for the optimal weather/crowd-avoidance combination (late May is very nice).

This island (the largest in the chain) boasts 11 of the 13 climate zones in the world, each with unique ecosystems and weather characteristics.
This is the best vacation spot I have been to.
Where else can you surf with whales?

Go for a cruise and surely you will enjoy more than one spot. Cruise lines carry their passengers to exotic places and it has tons of fun activities on board. They also have a great itinerary which is different on which cruise ship you decide to ride on.
