Since you did not specify which airline frequent flyer program you are interesting in, I will say that for any airline, you can either go to their website, click on the link for Frequent Flyer Information or it could be under Membership Program and fill out the online application. You could also call the main customer service phone number and ask to speak to a frequent flyer program representative for any airline.
