I did a pretty thorough search of HP.com looking for questionnaires and templates. Unfortunately, they don't publish much because their consulting services organization does all this for a fee.
I did find comprehensive desktop assessment tool kits available from Microsoft:
http://directory.partners.extranet.microsoft.com/providerdeliverables.aspx
You could probably extrapolate a server template from these tools.
If you are CISCO certified, you probably have tools for doing the network portion of the study.
Other areas would be storage and security assessments.
I used to do performance needs analysis to support enterprise server and storage sales, the majority of the information came from enterprise management, monitoring and performance tools. Information derived from the tools and observation of system performance was then confirmed through user interviews.

I am looking for an example of a Technical Site Survey Report/Assessment?
I have a client that needs me to go to their office and review their computer equipment, servers, etc. I want to give them a report of what I recommend that they do and what they should get (i.e. servers, printer, workstations, etc).
It would helpful to have an example of questions to ask and an example of the end result report would look like.
