Although most would actually use this on a linux server, I would suggest checking out the Asterisk project.

Asterisk can be configured as the core of an IP or hybrid PBX, switching calls, managing routes, enabling features, and connecting callers with the outside world over IP, analog (POTS), and digital (T1/E1) connections.

This is the realm of really complicated it involves flashing you router with new software. I'm not even sure if I know what your asking... Do you mean make you router a VOIP appliance or VOIP compliant or friendly or able to intergrate into other things. I've see stuff on dd-wrt site about VoIP flash mods. Don't know what they do but feel free to look.
