I have solved the problems that you're having in enterprise environments.
Basically, you need to do these things:
1)
If you're using VoIP, ensure that your upstream bandwidth is clean, clear and reliable. This is the number 1 reason that video conferencing services have problems. I'm sure that you guys have decent bandwidth where you are, but you may want to review your network topology and QoS (quality of service) rules to ensure that your network is configured, optimized and importantly consistently available.
Don't forget to look into the bandwidth of the people who are on the conference calls either. When working as part of a decentralized team where communication is key, it's important that their infrastructure (routers, cable, etc) is properly configured for high throughput as well.
2)
Use WebEx.

WebEx is the best. While WebEx does have various video and desktop sharing features, it can be used as a simple conference call solution. However, if you're using this in a team scenario (not just with potential customers), you will naturally gravitate toward whiteboard, desktop or full video sharing very quickly.
It's also possible to have some people call in (cell phone) and some people sharing on the same call. This comes in very handy.
WebEx has been created by and is ran by Cisco. They're sort of a big deal on the Internet ;) More then their name, Cisco has redesigned WebEx to leverage it's unified messaging platform and as a result is an exceptionally strong product.

In a previous Question / Answer, Mark Jeffrey mentioned that Mahalo doesn't like to spend a lot of money on resources if it doesn't have to. The $60 per month that WebEx costs is well worth it when you have a decentralized workforce.
3)
This may sound like overkill for a simple TalkShoe replacement, but communication can be a key competitive advantage.
So, while you can simply use WebEx with your existing voice infrastructure, consider contacting Cisco and allowing them to pitch you on Cisco's Unified Communications. This is a strong combination of voice and video that does a shockingly good job of replacing both your voice infrastructure and combining it with video.

In the end, switching this type of infrastructure over to Cisco will have a moderate startup cost but if you do the TCO math on it, usually (8 times out of 10) comes out ahead of any other VoIP or POTS setup and is much more functional.

What is the best conference call website/program to use?
We're looking at changing our conference call service. Right now Talk Shoe isn't the most reliable service. Anyone have suggestions? Ten Mahalo dollars to the person whose suggestion we use.

I have found that webmeeting has the best of 3 worlds. Audio, visual , and chat. You can teleconference in, ask questions to the moderator via chat while watching a powerpoint presentation. Enabled via remote connectivity, webmeeting connects everyone at the same speed regardless of your current internet connection.
