Have to tried BugZilla? Lots of major open source shops seem to gravitate towards it, which says something.

Yes, Bugzilla.
Unless by "web based" you meant on the Internet, on someone else's server.

Bugzilla you have to put on your own web server (Windows or Linux)

Atlassian has a free personal usage licence [url]http://tinyurl.com/5jjql7 /url] and many have moved from bugzilla to it. Have a look at the customer list [url]http://tinyurl.com/dldm3 /url] even Apache who made Bugzilla are now using it.

I have to say the best I've ever used is Trac (trac.edgewall.org). I've used it for quite a few versions, and it's integrated features (built-in wiki, subversion integration, bug tracking, workflow management, etc...) and it's plugin architecture can't be beat. I know Bugzilla has been the stallwort for quite a while, but I see Trac as a more user-friendly, modernized project management tool that is easy to setup and seems to be more lightweight than BugZilla in my opinion.
And also, open source and free!
