I have no idea what "Google Form" is,
but if your trouble is just a firewall
(i.e., nothing is wrong with the "Form"),
and, assuming you are doing this at work or school,
have you tried a different computer,
preferably, one in a completely different location,
such as a friend's or relative's or library, etc?

(WARNING: Attempting to bypass work or school firewalls
can get you fired or suspended, respectively).
