You can easily do this using Preview on the Mac (OS 10.5.6, not sure about older versions). Make a PDF of the cover. Open both the cover and document in Preview. Turn on the sidebar view. Drag the cover page (from it's sidebar view) to the document sidebar. Move pages if needed (in the sidebar). Save & done.

What you need is a program to edit PDF files.

I found a couple of free PDF editors

Or you could download a trial version of a PDF editor ( you can find a list here: http://www.tech-faq.com/Free-PDF-Editor.shtml )

A good options is Foxit PDF editor

Another alternative, as you said you didn't have the original .doc file anymore is to convert your .pdf back to the .doc file. (PDF to Word Document Converter)

Here is an easy to use online app:

http://www5.rptea.com/pdf/

Rename your cover page to 1.pdf and the rest of the doc as 2.pdf.
Zip and upload.

If you happen to have adobe Bridge, Photoshop or Acrobat Pro, these are all alternatives too.
