It looks like you most likely won't be receiving a tax form for Roth IRA contributions:
"Roth IRA contributions are not tax deductible and are not reported on your tax return. On the other hand, you do not have to pay tax on any qualified distributions, distributions that are a return of your regular Roth IRA contributions, or distributions that are rolled over into another Roth IRA. "
So since they don't get reported, I highly doubt you'll receive a tax-specific form detailing your contributions.

You will get a year end summary statement. But it does not make any difference in your tax filing. Go ahead and file your taxes. Your W-2 will not show any deductions related to the Roth IRA.
The W-2 won't show Roth IRA info unless it's a Roth 401(k) program.
