The coaches decide ultimately, but they both decide by their actions. Right now, McNabb is the starter because even if Vick was named the starter, the NFL Commish still has Vick under conditional reinstatement, which means he can train and practice, but he can't play in NFL games yet. It just makes professional sense that McNabb is the starter. But if he doesn't do well this season, and Vick gets completely reinstated somewhere before Week 6, there could be a change. But I completely doubt it.

No, the coaches will. Considering Vick has been out the the game for a while, that's likely to be McNabb. He's currently listed first in the depth chart.

No, Andy Reid will and it's been clearly stated that McNabb is and will be the starter. Most likely Vick will be used in the role of several plays per game as in option pass and run type of plays. That hasn't been officially stated by the Eagles' coaching staff but has been hinted at on several occasions.
