FIrst you have to complete & submit an Application For A U.S. Passport.
To do this you:
* Must apply in person at an Acceptance Facility or Passport Agency
* Must include the additional documentation required by Form DS-11
* Must not sign the application until instructed to do so by the Acceptance Agent
* Must provide your Social Security number

Second you have to submit proof of U.S. Citizenship
To do this you have to submit one of the following (they will be returned with your passport):

1) Previously issued, undamaged U.S. Passport
2) Certified birth certificate issued by the city, county or state
3) Consular Report of Birth Abroad or Certification of Birth
4) Naturalization Certificate
5)Certificate of Citizenship

Third present ID
You need to present acceptable identification at the time of application, one of the following:

1) Previously issued, undamaged U.S. passport
2) Naturalization Certificate
3) Valid Driver's License
4) Current Government ID (city, state or federal)
5) Current Military ID (military and dependents)

Fourth pay the application fee

Fifth provide two passport photos, they must be:

* Identical
* In color
* 2 x 2 inches in size
* Taken within the past 6 months, showing current appearance
* Full face, front view with a plain white or off-white background
* Between 1 inch and 1 3/8 inches from the bottom of the chin to the top of the head
* Taken in normal street attire

It'll take 30-45 days for processing and your passport to be mailed to you.

To get a U.S. passport fast, I would recommend using a passport expediting service. They would hand deliver your passport application to the U.S. Passport Agency and have it processed quickly. Some services, like RushMyPassport.com are able to have passport applications processed in 24 hours.

In order to expedite a new passport application you will need to complete the following steps.

Step 1: Contact and enlist a passport expediting service

Step 2: Gather your required documents, which would include:

- New Passport Application, Form D.S. 11 (electronically completed and printed from travel.state.gov)
- Two identical 2x2 color passport photos
- Government fee in the form of a personal check or a money order( fees vary,and do change. Confirm appropriate fee with your passport expediting service)
- Your original proof of citizenship (original/ certified copy of your birth certificate, or naturalization certificate)
- Photocopy of your driver's license or state issued ID
- Authorization letter that authorizes the expediting service to act as an agent on your behalf
-Travel Itinerary ( depending on how soon you plan to travel it may be required that you submit a travel itinerary showing planned international travel)

Step 3: Visit a Passport Acceptance Agent at your local post office or County Clerks office to have your documents verified and sealed ( placed in a sealed envelop to be opened by the Passport Agency

Step4: Ship your documents to your passport expediting service following their instructions

Step 5: Await your new passport.
