Definitely the quality and the look. A designer purse is made with better quality materials and stitching, and the designs are much nicer. However, I don't think that designer purses should be as expensive as they are. I knew a woman that bought a $3,000 Coach purse -- that's outrageous! But, you don't have to get the outrageously priced designer purses. There are some less expensive designer purses that are very nice. For example, I love the Nicole Miller purses at JC Penny that you can get for around $40-$60. And Macy's has Liz Claiborne purses that your can get for about the same price. And if you go to after Christmas or New Year's sales, you can get them at 50% off. That's typically what I do at the end of every year.

Yes not all cheap things has less quality than the designer stuff ones but you can be sure that you could use it longer than the cheap ones.
For instance, I used buy super cheap flip-flops years back and none of them lasted. While when I bought Havaianas which is 700% more expensive than the cheap ones I used to buy, it lasted for 4 years.
And as @rishiku said, yes, the name itself is a plus. Especially to those who are brand conscious and trying hard people... sorry for the term.

Quality. Yes designer items could be expensive, of course you're paying for the name itself, plus the quality materials they use for you item. They wouldn't use something that wouldn't last for long right? Yes not all cheap things has less quality than the designer stuff ones but you can be sure that you could use it longer than the cheap ones.

Less people have it, making it less likely you will have that awkward moment when you leave a party and realize that wasn't your purse you just walked off with. Also, Walmart just doesn't have that good of purses they are always falling apart and generally either look to preteen or to old lady.
