I don't know where you live, but in many urban areas there are food-buying clubs. I eat farm-raised meat, and various of my friends have been to the farm where we got the latest cow that we split among ourselves, but I haven't. On the other hand, I've been to the farms where we get our chickens, eggs, milk, and pork.
So yes, someone you know well personally or you yourself need to go to the farm regularly (so you know they didn't just tidy up knowing a special visit was coming)--you should be able to drop in pretty much unannounced (at least with minimal warning).
Try finding a local chapter of the Weston A. Price foundation, or check Eat Wild. You can also try hanging out at farmer's markets and talking to people who are shopping at stands with signs that say "grass fed", "pasture fed" etc. or look for the local Weston A. Price chapter.

What are my options for buying poultry and beef? I don't want to eat anything factory farmed or raised inhumanely.
I had no idea things had become so bad. Apparently, even poultry marked as "free range" or "cage free" doesn't really mean what we think it does: http://www.farmsanctuary.org/issues/factoryfarming/poultry/
I've been eating less meat, but I'm still not planning on going vegetarian. How can I eat meat but know that it will be safe, hormone/antibiotic-free, disease-free, etc.? Will it take driving out to a farm myself?
