You need a good topic. Talking about tech one week and then snowboarding the next is a good way to lose listeners. Even if the basic concept is what i did this week/month you should really limit the topics to basic subject. Off topic comments about things that are tangentally related are ok on occasion but not as the rule. (Of course if you are really entertaining with them could be a good topic it self)
Have a consistant format. Choose a flow of the podcast and stick with it. If changes need to be made make them incrementally overtime. Hopping around like you chose the format at random is a sure way to turn off your listeners.
Be regular. If you say you are going to have a daily podcast make sure you are basically a daily podcast. Missing a day or 2 a month may be acceptable but irratically releasing new podcasts is a great way to drive away listeners. If it is a weekly or biweekly or monthly podcast its is even more important. Set your day of release and stick too it. I got into a couple of really cool podcasts only to find out their "Weekly" Podcast was really when ever i feel like doing one and got tired of going to find no new podcast this week.
Avoid annoying noises. Remember that alot of people are listening to your podcast while doing something else, be it the commute to or from work or while working on something at work, or making dinner. While an annoying sound may grab their attention if it happens to often they are just a likely to decide to stop listening as to pay attention to what you say after it.

What initial steps are needed to create a successful podcast?
I know that plan, plan and plan are the best steps; however, I am looking for the communities input on how to do it...

I have found that making the editing process simple to be very useful. If you can cut down the time of post-editing, the more time you have to do other things.
