My mothers horse has a bald patch on her back down the spine. It's still winter here in Alaska. How can she tell if it's fungal?

There's no way you can really tell without a test. Many skin infections have similar symptoms and your horse needs to be tested for mange as well as bacterial and fungal infections.
Call the vet.
