My little brother accidentally stepped on my foot and now it is bleeding under my big toes nail. Should I go to the hospital?

Most likely, the skin at the edges of the toenail is split, causing the bleeding. If you can control the bleeding and get it to stop within a reasonable time, then there should not be a need to go to the hospital or critical care clinic. In the next day or so, you may experience bruising under the toenail, and it is possible that you will eventually lose the toenail.

Clean the wound(s) well by flushing with clean, warm water. Use hydrogen peroxide, or a disinfectant spray to disinfect the wound (this may sting). Consider covering the wound with an antibiotic ointment and a self-stick bandage.

Things to watch for:
1) A red "line" moving up your toe and/or foot over time. If you see this, go ~immediately~ to the Emergency Room of your nearest hospital.
2) A red, scaly, itchy rash on the toe or foot. If you see this, go to your physician as soon as possible.
3) If your discomfort lasts more than three days, go to your physician to have your toe checked out.

Consider putting an ice bag on the toe for the first 24 hours to help reduce swelling and to help with pain.

If the blood is trapped under the toenail, complications, especially a necrosis, could develop, which is the decaying of the body tissues. The skin under the toe will become green or black, which can be mistaken for dried blood. Gangrene can set into the decaying body tissues of the big toe. Antibiotics can be used if the gangrene is caught in time; otherwise, amputation is the only solution. The reason for the amputation is to prevent the gangrene disease from spreading into the brain, heart and liver. The only way to prevent death from happening as a result of gangrene is to go to the hospital for examination and treatment. In the picture, it's gangrene, which started out as a little scratch on the toe, and left untreated.

I wouldn't go to the ER, you are going to spend hours waiting while they treat and seethe sicker people. If you are concerned I'd suggest either visiting your family Dr. or a walk in clinic. Your family Dr. will give you an appointment and the walk in clinic treats you in the order your arrive so you aren't going to spend hours warming the chairs in the waiting room.

Painful experience, my son fell off his bike last week, my wife took him to the ER, 6 hours later he was home with two stitches and a bandage on his arm.

put plenty of ice on it. it will go black for a while but i think you'll survive

I had that once, went to the family doctor because it hurt like hell. He make a little hole in my toe nail to let the blood come out to relieve the pressure. Pain was instantly gone and I didn't loose my nail.

well I'll tell that something like that happen to me when i was young and to be honest my mother didn't take to the hospital on time and my nail turned green and it fell off after the infection i got so my biggest advice to you is to yes go to the E.R as soon as possible

not unless the bleeding is profuse and won't stop or he pain is abnormally severe. wash it in mild soap & water and apply pressure to stop the bleeding. once i shut my finger in a car door, it bled but stayed UNDER the nail. the nail turned black but no blood came out. the pressure it caused was so painful , it extended all the way up my arm to my shoulder! after about 8 hours had passed , i could no longer stand it and went to the er. the Dr. came out w/a little drill - much to my horror! he made a tiny hole which didn't hurt at all! ONE drop of blood came out and the pain was gone.
