my buddy just got asked if he was "m-o-t" by an italian-looking guy ... what does that mean?

The "Italian-looking guy" was possibly Jewish. If so, he was colloquially asking your friend if he was Jewish as well, since M-O-T sometimes stands for "Member of the Tribe".
