Wait until the bill for the ambulance arrives. Next, immediately call the "claims" phone number on your insurance card. If it's an automated system, listen for the "file a claim" prompt. If you can't understand or use an automated system, simply press zero several times until it asks you to hold for a representative. Tell them you have to file a claim for ambulance charges. The rep should walk you through what information they need. This might include your insurance policy number (on your card); the ambulance bill; and personal info, such as address or ssn. Finally, they will process your claim. You may have to make the 1st month's payment on the bill, if the claim has not been settled by the time the first payment is due. If ambulance charges are covered in your policy, you should receive a check to reimburse for anything you have had to pay.
