Her main accusers say she just didn't have the background or knowledge to effectively investigate. She says she's just mid-level management, that she oversaw lawyers and had plenty of folks at the SEC overseeing her.
My take, for what it's worth -- no one took it seriously enough that such a well-known figure in the industry would devise such a big scam...and therefore no one dug deep enough until there was finally just too much evidence to ignore...
