If you requested payment during August and didn't receive it October 15th.. did you submit a W-9?

3. Did you need to fill out a W-8 or W-9 form? You will need to fill out a W-8 form (if you are not a US citizen) or a W-9 (if you are a US citizen) before your first payment can be processed. If Mahalo finds any signs of abuse/cheating/gaming the account holder will be notified.

Maybe there is a possibility that you don't find the record of transfering in paypal because the record has been deleted,because paypal always delete the history record after a month,it have been 2 months+ from august to now,so I doubt it.you may contact paypal for more history record of transaction,or contact the stuff of this site if you make sure that you don't receive it. @mike is a stuff in this site.good luck to you.
