Yes this is required. I'm from the Philippines too and last September's my first cash out here. I received the payment by October after filing up a w9 form and sending it back to a Mahalo personnel.
This IRS form is for the people outside US to be used by Mahalo to request your taxpayer identification number.
It's pretty straight-forward too that you won't have difficult time filling it out, but if you want a guide or something to fill this up, go to this page:

After filling up, you can fax this or scan it and send it back to them.

You should also write your username on the form (so Mahalo know which account is associated with it.
The Minimum payment request is M$40 and if you request a payment by the end of the month it will be paid into your paypal account around the 15th of the next month.

Yes.. The form is required for Mahalo to authorise payments from non US citizens, US citizens have to fill in a w9 instead. Details on how to fill in the form can be found here
